Coronary artery disease is considered to be a major cause of mortality and morbidity in the developing world. It was shown that aortic root pathologies such as aortic stiffening and calcific aortic stenosis can contribute to the initiation and progression of this disease by affecting coronary blood flow. As a consequence the coronary blood flow and velocity profiles can be altered which could contribute to the development of coronary artery disease.
We present the evolution of a 3D Fluid Structure Interaction model of the aortic root incorporating the leaflet apparatus and the coronary structures down to the second bifurcations (small coronary arteries) that was developed over the years. This global model allows to assess to quantify the impact of several hemodynamic variables (aortic stiffening, viscoelasticity, hyper-elasticity) on the coronary perfusion.
We also present an initial strategy to develop a Digital Twin (DT) of the aortic root based on PINNs (Physics Informed Neural Networks) to provide physicians with a quasi-real time decision support tool for emergency surgical cases. The DT will also merge clinical pressure measurements (data driven) and the physics of the phenomena.
